If you would like to inquire about making a reservation with Oahu Diving please kindly fill out the below form including your mobile phone number and email. Once we agree on a dive charter for you please also let us know the following information;
We do not accept reservations from people who do not have a direct contact mobile/cell phone.
- We need to know each participants age, name, height, weight shoe size and age.
- We do not accept reservations from people who do not have a direct contact mobile/cell phone.
- Please read the dive/liability release form that applies to you.
- Please print, fill out the forms and bring with you. (we do have these forms at the boat if you can not do this)
- The desired day you wish to participate. A few dates are better for us in case we are fully booked on that particular requested day.
- You must wait 18 hours after your last dive before you fly in a plane. You may dive right away after arriving in Oahu.
- At this time we do not accept credit cards or personal checks
- We only accept bookings 30 days or less from the time you will be here in Oahu.
